You can order a replacement card on our website www.ILostMyCard.com. Simply select your certification state, and course, add your name and date of birth and you will be able to order with a credit/debit card and receive a temporary copy immediately via email. The permanent card will arrive via mail in 3-5 weeks. Please note that the envelope the card arrives in will be a plain envelope. If the card is not received within the allotted time (5 weeks), please reach out to us immediately.
If you cannot find your course and state at ILostMyCard.com, then we are probably not authorized to provide replacements. You may need to contact your state’s issuing authority to get a replacement or find out who can supply replacements.
If you are able to find your certificate and state at ILostMyCard.com, but are unable to bring up your record, we either do not have your record (in which case you will need to contact the company that provided your course) OR we only have a partial record. This often occurs when an in-person class or a competitor’s course is taken, and the record is then forwarded to us by the state to provide replacements. Please call our support line (see below), so that we can assist you to get a replacement card.
