Name Change/DOB:
- Once you finish registration, we can only change your name or date of birth if you provide us with a government-issued ID.
- Please provide us with a government Issued ID: Passport, Driver's license, or Divorce/Marriage certificate for our team to make the correction(s).
- Please note, that if you have already completed the course, you may need to reach out to your state or provincial agency for their records to be updated as well.
- Once we receive your document, we will be able to update your record. From there you will be able to order a replacement card from ilostmycard.com.
